- A completed Three Affiliated Tribes - MHA Nation employment application
- Two forms of ID, such as
- A valid driver's license
- A state-issued ID
- Social Security card
- Passport
- Birth Certificate
- Proof of your most advanced education
- High school diploma
- College degree
- Transcripts
- GED
- Certificates
- If applicable, Certificate of Degree of Indian Blood (CDIB) or Tribal ID
- If applicable, Certificate of Release or Discharge from Active Duty (DD-214)
- If applicable, Official Registration Acknowledgment Letter (Selective Service)
Please Note: Applications must be complete with all required documentation (2 forms of ID front and back, and proof of education). IDs need to be in color. Please scan and email as an attachment.
