To be considered you must submit a complete application that consists of the following:

  1. MHA Nation Employment Application
  2. HS Diploma/Official College transcripts
  3. 2 forms of ID (all of our positions require a valid drivers license)
  4. A copy of any relevant certifications and/or licensures you may have
  5. Childcare Questionnaire (background purposes)
  6. I.H.S. Addendum form

Please Note: If requirements are not met, i.e. submissions of a resume in lieu of a Three Affiliated Tribes application, or not including a required certification or document, your application will not be reviewed and will be disqualified. Applications will not be returned and kept for Elbowoods Memorial Health Center records only. We do not accept copied, faxed, or scanned applications.

Indian Preference Will Apply: In accordance with Three Affiliated Tribes policy, priority in selection will be given to qualified applicants who provide proof of eligibility for “Indian Preference”.

Veterans Preference Will Apply: In accordance with Three Affiliated Tribes policy, priority in selection will be given to qualified applicants who provide proof of eligibility for “Veteran Preference”.
